Apple Watch Ultra 4 may get complete redesign with advanced blood pressure monitoring

Apple’s next flagship wearable could mark a major shift for the company’s health-focused device strategy. The Apple Watch Ultra 4 may feature a complete redesign alongside enhanced blood pressure monitoring capabilities, according to a new report from DigiTimes.

The timing is significant as Apple faces increasing competition in the health wearables space from companies like Samsung and Fitbit, while also dealing with regulatory challenges around medical device approvals. A major redesign could help Apple maintain its market leadership position.

The blood pressure feature will apparently use the optical heart-rate sensor on the device’s back to analyze how blood vessels respond to each heartbeat. When an abnormal pattern is detected, the watch will send alerts to users. The feature is currently under FDA review, which highlights Apple’s continued push into clinically validated health monitoring.

This appears to build on Apple’s existing Hypertension Notifications feature introduced with watchOS 26 last fall. That feature also uses the optical heart sensor to analyze blood vessel responses, but over 30-day periods. The new implementation seems to represent a more refined or clinically validated version of the underlying hardware capabilities.

The physical redesign could be equally significant. Supply chain sources point to exterior design changes including eight sensors arranged in a ring pattern on the device’s back. DigiTimes describes this as a “full redesign” with a “significant upgrade to sensing functions.”

Market observers believe these changes could boost Apple Watch shipments by 20% to 30% compared to 2025 levels. The sensor upgrades are expected to particularly benefit Taiwan-Asia Semiconductor (TASC), Apple’s exclusive supplier of sensor components, with large-volume orders anticipated as early as July.

Apple’s health monitoring ambitions extend beyond blood pressure. The company’s next major target is noninvasive blood-glucose monitoring, a capability it has been pursuing for years while awaiting government approval. Success here could be transformative for diabetes management, affecting millions of users worldwide.

The Apple Watch Ultra 4 is expected to launch alongside several other major Apple products in fall 2026:

  • Apple Watch Series 12
  • iPhone 18 Pro and iPhone 18 Pro Max
  • Foldable “iPhone Ultra”

This product lineup suggests Apple is preparing a major hardware refresh across multiple categories. The inclusion of both a redesigned Ultra watch and a foldable iPhone indicates the company is willing to take bigger design risks as smartphone and wearable markets mature.

The blood pressure monitoring feature, if approved by regulators, could position Apple’s wearables more firmly in the medical device category. This shift has both opportunities and challenges, as medical device regulations are more stringent but the market potential for validated health monitoring is substantial.
